How Bicillin L-A Affects PatientsPenicillin G is an antibiotic used to fight bacterial infections. It is part of a group of antibiotics known as penicillins. It works by stopping the bacteria from forming walls that protect and help them grow, and is effective against both gram-positive and gram-negative bacteria. Penicillin G is resistant to most beta-lactamases, which are enzymes used by bacteria to break down antibiotics.
How Bicillin L-A works in the bodyPenicillin G works by attaching to proteins inside the bacterial cell wall, stopping the cell wall from finishing its growth. This causes the cell to break apart, using enzymes that are naturally found in the cell. Penicillin G may also block an enzyme that helps keep the cell from breaking down.

When to interrupt dosage

The prescribed dosage of Bicillin L-A is contingent upon the diagnosed condition, such as rat bite fever, Haverhill fever and Actinomycosis. The amount of dosage is dependent upon the technique of administration (e.g. Intramuscular; Intravenous or Injection, suspension) as illustrated in the table below.

I had strep throat and was given amoxicillin, but it didn't seem to help after four days. So my doctor gave me this bicillin shot instead and I felt SO much better almost immediately! Not only did my sore throat get better, but my body aches went away too. The shot stung a bit, but it wasn't bad at all compared to how I was feeling before.

I found myself in the ER early Saturday morning with swollen glands, difficulty swallowing, and overall neck pain. The doctor diagnosed me with strep throat and gave me the option of an IM antibiotic along with a steroid pill. I felt better hours later, but experienced some stomach pain during the night and following day. The shot hurts, but it's better than having to swallow giant pills for 10 days.

I was given this injection at the ER after a strep test came back positive. I felt much better after 24 hours, although the injection site was sore for a couple days afterwards. I was also given oral steroids to relieve swelling in my throat. My family doctor prescribed 10 days of amoxicillin about 18 hours later; however, I have since developed diarrhea and feel like vomiting.

Bicillin L-A for Early Stage of Syphilis with No Symptoms
I had a pretty bad experience with this treatment. I was given one shot for exposure to syphilis, and a few hours later I started running a fever and feeling flu-like symptoms. My friend found me at 1am in the ER covered in bags of ice, with a 105 degree fever and unresponsive. The Jarisch-herxheimer reaction almost killed me, and I spent 36 hours in the hospital under observation.

Cleared up the strep, but caused me to have a seizure (which I have never had before). It was a horrible experience. I couldn't get out of bed the entire next day. Felt like I had been hit by a truck. The vaccination site was extremely sore for a few days, and the shot in the buttocks was not a fun experience either...very painful.

I had a really terrible experience with this treatment. The shot was painful and didn't do anything. I went back to the doctor and argued for something else until they finally prescribed Clindamycin. Within 6-12 hours I could tell it was working better.

Intrapleural Enzyme Therapy for Pleural Infection

The goal of this clinical trial is to find out if giving certain medications once a day works just as well as giving them twice a day to treat infections around the lungs (called pleural infections). These medications-tissue plasminogen activator (tPA) and deoxyribonuclease (DNase)-are placed through a chest tube into the space between the lung and the chest wall to help clear out the infection. The investigators are trying to learn: * Does using the medicine once a day work just as well as using it twice a day? * Are there any differences in outcomes between patients who get the medicine once a day versus twice a day? * Does more or less fluid remain in the chest (seen on a chest x-ray) depending on how often the medicine is given? Participants will: * Have an infection around their lung and will already be getting normal hospital care, including a chest tube to drain the infected fluid around their lung. * Be asked to give permission to join the study. * Be randomly chosen (like flipping a coin) to get the medications either once a day or twice a day through the chest tube.

Rapid Test for Syphilis

The goal of this clinical trial is to learn if a finger-prick blood test for rapid diagnosis of syphilis infection can be implemented in a low-barrier prenatal clinic for people at risk of syphilis in pregnancy. The test gives results in 5 minutes and also tests for HIV. The main questions the study aims to answer are: * How often do pregnant people at risk of syphilis agree to the rapid test? * How well do healthcare providers perform the rapid test? * Does the rapid test speed up diagnosis and treatment of syphilis in pregnant people and their sexual partners? Participants will: * Give feedback about why they did or did not want to take the rapid test, and what their experience was taking the test; and * Share information about their health, pregnancy, and syphilis treatment (if applicable).

Decision Support for Lower Respiratory Infections in Children

Eliminating inappropriate antibiotic use in pediatric lower respiratory tract infections (LRTI) is the central focus of this research. LRTIs (pneumonia, bronchiolitis, and infection-related exacerbations of asthma) account for nearly one-third of all emergency department (ED) visits and 40% of all infection-related hospitalizations in US children. LRTIs also account for more antibiotic use in children's hospitals than any other condition, despite most LRTIs being viral in nature. Inappropriate antibiotics are associated with substantial adverse effects. Accordingly, national guidelines strongly discourage routine antibiotic use for bronchiolitis and acute asthma and argue for significantly reducing antibiotic exposure (initiation, spectrum, and duration) in pneumonia. To address the problem of inappropriate antibiotic use, hospital-based antimicrobial stewardship programs (ASPs) are now common nationwide, and these programs have demonstrated effectiveness in some hospital settings. Unfortunately, traditional ASP approaches do not translate well to the fast-paced and unpredictable ED environment, and hospital-based ASP resources are finite and not always immediately available. Clinical decision support (CDS) embedded within the electronic health record (EHR) is a strategy that could address the ED antibiotic stewardship gap. Informed by a deep understanding of the key facilitators and barriers to using CDS to support appropriate antibiotic use in ED and hospital settings, the investigators have developed two stewardship-focused CDS interventions for pediatric LRTI. The overarching goal of this research is to rigorously evaluate the implementation and effectiveness of these CDS tools, alone and in combination, against usual care only in a pragmatic randomized clinical trial at 3 U.S. children's hospitals.

Doxycycline for Syphilis

The goal of this clinical trial is to learn if doxycycline taken as 200mg daily for 14 days is effective to treat early stage syphilis. This is different from how doxycycline is typically used for syphilis because the full doxycycline dose will be taken at the same time of day, rather than split up into a twice daily regimen. Lab data support that taking the medication as a single daily dose should be effective as treatment, but it has not been studied clinically. The main question this study aims to answer is: Is doxycycline taken as a single daily dose of 200mg for 14 days an effective treatment for early syphilis based on a combined outcome of clinical improvement and blood test improvement? Participants will: 1. Take doxycycline 200mg daily for 14 days 2. Submit oral and rectal swabs that test for syphilis bacteria every other day for 2 weeks, returned by mail 3. Complete 2 brief online surveys over the first 2 weeks 4. Return to the clinic for an interview and blood draw every 3 months for a maximum of 3 study visits, including the first visit The investigators will compare the percentage of participants in the study who have response to treatment by 6 months to that of persons who have received standard (CDC-recommended) regimens. To do this, the investigators will calculate response percentage estimates following a shot of long-acting penicillin or 14 days of doxycycline 100mg twice daily from 60-person samples from the sexual health program's records.

Rapid POC Testing for Syphilis

The purpose of this study is to offer routine lab-based and rapid point of care (POC) syphilis diagnostic testing in the emergency department (ED) and obstetrical (OB) triage , thereby increasing screening and treatment initiation for pregnant patients without prenatal care or with no documented syphilis results during the index pregnancy in other to increase rates of full treatment to determine if the implementation of a robust linkage to care program for pregnant patients with positive rapid POC results will more likely result in complete treatment for all stages of syphilis and adherence to recommended maternal and newborn longitudinal follow up and to assess factors that contribute to treatment completion after implementation of the intervention will also be analyzed to tailor efforts targeting social and healthcare navigation factors that affect health equity; including poverty, insurance, health literacy and others.

Rapid Test for Syphilis

Due to the recent resurgence of infectious syphilis in Canada and the changing epidemiology of the disease to involve heterosexuals and females of child bearing age leading to a record number of congenital syphilis cases and stillbirths, the demand of a rapid test such as POCT that can be used at the place and time that front-line public health workers meet the at risk population is getting louder. Indeed during a number of federal, provincial and territorial meetings on the control of syphilis outbreaks in Canada, requests for POCT to detect syphilis infections have repeatedly come up. However, before a POCT can be widely used, research to find out if it is accurate and reliable is necessary. This study is designed to test the performance of the MedMira Inc. Multiplo® Complete Syphilis (TP/nTP) Antibody Test (POCT) (MedMira Inc., Halifax, Nova Scotia) in an urban clinic (Sexual Health Clinic) in Ottawa, Ontario, Canada and compare its performance in parallel with the usual testing method (the gold standard, i.e. conventional syphilis serology testing).

Portable Dynamic Chest X-Ray for ICU Patients

Dynamic digital radiography (DDR) is a new advanced version of chest radiography that captures dynamic images at a rate of 15 frames per second. It is coupled with an analytical software that allows it to provide more advanced measures of lung motion, ventilation, and perfusion compared to traditional chest radiography. While implementation of DDR fixed machines are beginning elsewhere in the US, this trial involves the first applications of an FDA-approved portable DDR machine, for use at the bedside in the ICU. The goal of this clinical trial is to determine the feasibility and safety of portable DDR technology in the ICU, as well as to evaluate the improved clinical diagnostic value of the portable DDR system over current standards of care. Participants will receive one to three sets of DDR images, which will then be compared to their clinical gold standard exams (such as chest x-rays, CTs, or VQ scans) to assess and improve the precision and accuracy of measurements such as diaphragmatic motion, lung movement, and perfusion.
