Clindamycin

Clindamycin is a type of antibiotic that is used to treat serious infections caused by susceptible microorganisms. It is also used to treat acne when applied to the skin. It works by targeting certain bacteria and protozoans. Clindamycin is a semi-synthetic drug that is derived from lincomycin, which was first found in soil in Lincoln, Nebraska. It is more effective than lincomycin and is often used instead.

How Clindets Affects Patients

Clindamycin works by stopping the growth of bacteria. It needs to be taken every six hours to make sure enough of the drug is in your body. There is a risk of getting an infection called Clostridium difficile associated diarrhea (CDAD) from taking clindamycin, which can range from mild diarrhea to fatal colitis and can happen up to two months after you stop taking the antibiotic. Because of this risk, clindamycin should only be used for serious infections that cannot be treated with less dangerous antibiotics. How Clindets works in the body

Clindamycin works by blocking the ribosomes in bacteria from making proteins. It does this by mimicking the shape of molecules that are used to make proteins and preventing them from binding. We don't know exactly how it works to treat acne, but it is thought to be related to its ability to stop the growth of Propionibacterium acnes, a type of bacteria that can cause acne.

Clindets Toxicity & Overdose Risk

The lowest toxic dose of clindamycin in mice and rats has been found to be 2540 mg/kg and 2190 mg/kg, respectively. Although no overdoses have been reported yet, possible symptoms could include stomach pain, nausea, vomiting, and diarrhea. One 3-year-old child was given a dose of 100 mg/kg per day for 5 days and only had mild stomach pain and diarrhea. Activated charcoal can be used to remove the drug if it was not absorbed yet. Dialysis is not effective in this case. In the event of an overdose, general supportive measures should be taken.

AI-DBT for Suicide Prevention in HIV/AIDS

One in four older persons living with HIV/AIDS (PLWHA) report at least one suicide attempt in their lifetime, and the risk for death by suicide is 100 times higher in PLWHA than in the general population. Currently, there are no behavioral interventions that specifically address suicide prevention for older PLWHA, despite their unique biopsychosocial and structural risk factors. Through this work, investigators will adapt Dialectical Behavior Therapy, an evidence-based intervention for suicide prevention, for patients with PLWHA to be delivered by an AI-powered conversational Agent developed by our industry partner, Empower Health. Investigators will then pilot test the feasibility, usability, acceptability and preliminary efficacy to improve self-efficacy to manage negative emotions in n=50 older adults living with HIV/AIDS.

Systemic Corticosteroids for Pneumocystis Pneumonia

The HOW LONG trial is an international, multicenter, Phase IV randomized clinical trial evaluating the optimal duration of adjunctive systemic corticosteroids in immunocompromised adults with severe Pneumocystis jirovecii pneumonia (PCP) who demonstrate early clinical recovery. Participants who no longer require supplemental oxygen by day 10 of corticosteroid therapy are randomized to discontinue corticosteroids at day 10 (or hospital discharge, if earlier) versus continue corticosteroids for a total of 21 days. The trial assesses whether earlier discontinuation reduces steroid-related complications while maintaining clinical outcomes.

Decision Support Tool for HIV Treatment

This study is testing software designed to help healthcare providers choose the best HIV treatment combinations for their patients. HIV medicines, known as antiretroviral therapy (ART), can be complex to manage because the right regimen depends on many factors-such as drug resistance, other health conditions, and medication schedules. Many people with HIV are cared for by general clinicians who may not have access to HIV specialists, which can make treatment decisions more challenging. In this study, healthcare providers will use patient cases to compare standard HIV treatment resources with a new clinical decision support tool that gives evidence-based ART recommendations at the point of care. The investigators hypothesize that using the tool will help providers select treatment plans that better match clinical guidelines, make decisions faster, reduce mental effort, and increase overall satisfaction with the prescribing process.

myPACE+ Algorithm for Hypertrophic Cardiomyopathy

This will be a two-arm investigator-initiated randomized controlled study of patients with nHCM and LV ejection fraction ≥50% and NYHA II-III symptoms, normal intrinsic conduction system and pre-existing suitable dual-chamber implantable cardioverter defibrillators (ICD) systems. Patients will be randomly assigned to either personalized accelerated pacing (using the myPACE+ algorithm with mono-fractional exponent) or usual care groups. At baseline and after 3 months of pacing all patients will undergo a CPET, echocardiogram, blood work for NT-proBNP levels and complete the KCCQ-OSS and HCMSQ, questionnaires. The investigator team hypothesizes that personalized accelerated pacing will be safe and improve symptoms and heart-failure related quality of life, physical activity, pVO2, biomarkers (i.e. NT-proBNP), diastolic parameters and cardiac structure.

Amoxicillin Challenge for Penicillin Allergy

The goal of this clinical trial is to learn about dosing when testing to see if a penicillin allergy label can be removed from adults that had been labeled as "penicillin-allergic" previously. The main question it aims to answer is: \- In penicillin-allergic patients that are at low risk of having an allergic reaction, is a one-dose oral challenge with amoxicillin (a penicillin-based antibiotic) as safe and effective as a two-dose oral challenge? Participants will, after being identified as having a low-risk penicillin allergy, be administered oral amoxicillin in a controlled setting and then monitored for an allergic reaction. Researchers will compare participants that took one dose of amoxicillin to participants that took two doses of amoxicillin (a small dose and then a larger dose) to see if either group was more likely to develop an allergic reaction.

The EXCITE-HCM study is a randomized, controlled, blinded clinical trial designed to evaluate the effect of moderate intensity exercise training versus usual physicial activity on the improvement of HCM-related symptoms and cardiac function. About 70 participants will be recruited and randomized on a 1:1 ratio to either moderate intensity training or usual physicial activity interventions. Patients will be followed during a period of 24 weeks and assesesments as physical examination, questionnaires, 12 lead ecg's, biomarker levels, echocardiogram, Cardiac Magnetic resonance, PET and CPET will be performed to evaluate their response to the intervention.
