This trial is for clinicians (nurses, care managers, social workers, physicians) working at the pilot site and patients aged 18 or older who are admitted to the ICU for intensive care services at one of the three pilot sites during the testing phase.

This is a single arm, open trial that will be conducted in parallel in 3 separate ICUs. Each site will receive the SET-to-MEET intervention.

This trial includes the following eligibility criteria:- Only healthcare providers who work or provide services at the pilot site can participate.You are at least 18 years old and have been admitted to the intensive care unit (ICU) at one of the three selected hospitals during the pilot testing phase.
